Madera Community Hospital has a Financial Assistance Program (Charity Care Program) to aid local
residents and patients to receive discounted or free healthcare services. Our program first helps
determine if you qualify for one of the many State and Federal public benefit programs available.
If you do not qualify for a program, our staff will assist you in applying for financial assistance.

Please submit the completed application, along with the requested documentation,
to the Madera Community Hospital Credit Office at 1250 E. Almond Ave., Madera, CA 93637. The
completed application can also be faxed to the Credit Office at (559) 675-5574. Please contact the
