Benefits
Madera Community Hospital offers its employees a variety of
benefit options. These descriptions are brief summaries, if you have any
questions or would like additional information, please contact the Human
Resources Department at (559)675-5555 extension 5825.
As a full-time (working minimum of 72 hours per pay period)
or part-time (working minimum of 40 hours a pay period) employee you are
eligible for the benefit options listed below. Insurance coverage begins the
first day of the month following 30 days of employment. Per Diem employees are
eligible for certain benefits.
Medical/Dental/Vision Plans
Employees can enroll themselves as well as dependents
(spouse and children). Employees can choose from two medical plans. Both plans
are Blue Cross PPO options and include vision care, dental and prescription
coverage.
Health/Dependent Care Reimbursement Account
Employees may elect to enroll in the Cafeteria Section 125
Plan. Enrollment is effective the 91st day of employment or during
the open enrollment period for the following calendar year. Employees can set
aside dollars on a pre-tax basis to pay for out-of-pocket medical and dependent
care expenses.
Basic Life Insurance/Accidental Death & Dismemberment
Full-time and part-time employees are eligible for basic
life insurance and accidental death and dismemberment coverage at no cost.
Employees may also enroll dependents (spouse and children) on the dependent life
insurance program providing they pay the monthly family premium.
Supplemental Life Insurance
Employees can choose to purchase supplemental term life
insurance. This premium is age rated. Rates and more information can be
requested from the Human Resource Department.
Other Insurance
Madera Community Hospital does offer voluntary benefits to
all employees. These include: Short Term Disability, Long Term Disability,
Cancer Coverage, Term Life Insurance, and Universal Life Insurance. If you are
interested in learning more about these plans, please contact the Human Resource
Department.
Workers’ Compensation
State law requires MCH to provide Worker’s Compensation
coverage to its employees.
State Disability Insurance (short-term) AND Paid Family
Leave Benefits (EDD)
California employees are covered for up to 52 weeks for any
illness or injury, either physical or mental, that prevents one from doing
regular or customary work. Disability also includes elective surgery,
pregnancy, childbirth, or related medical conditions. Paid Family Leave is also
available to employees. Paid Family Leave provides employees some compensation
to care for parents, children, spouse, and domestic partner or to bond with a
new child. Paid Family Leave provides benefits but does not provide job
protection or return rights. PFL offers up to 6 weeks of benefits in a 12-month
period.
Retirement
MCH Profit Sharing Retirement Program
Each employee who has worked at Madera Community Hospital
one full year, is 21 years of age, is full time or part time, worked 1000 hours
during the year and is employed at the end of the year, will be enrolled
automatically in the MCH Retirement Plan. Retirement Contributions are
contributed to the employee’s account at a percent of earnings of employee,
added to enrolled employees’ account each pay period. Employees must meet
vesting requirements before monies are provided from account to employee at time
of retirement, early retirement or termination. Full Vesting is at end of seven
years. Employees are not required to contribute to the profit-sharing
retirement program to qualify.
TSA Programs 403(b)
Madera Community Hospital provides all employees with
investment opportunities to assist them in saving for future needs and
retirement through the Tax Sheltered Annuity Savings Plan (TSA). All employees
are eligible to participate in a TSA program immediately upon hire.
Participation is available through payroll deduction. Contributions to a 403(b)
reduces taxable income per IRS regulations. Madera Community Hospital works with
VALIC, Lincoln National, Zahorik Company, Edward Jones Investments and American
Express Financial Services, all can assist you in establishing a TSA.
Time Off
Paid Time Off (Vacation, Holiday, Sick)
All regular full-time and part-time employees working at
Madera Community Hospital will accrue Paid Time Off beginning with their date of
employment. Eligible employees will be credited with the applicable accrual
rate for productive hours and benefit paid hours (regular, straight time,
overtime, doubletime, call back, vacation (PTO), sick (EIB), jury duty, limited
duty, bereavement, etc.), but excluding on-call.
Extended Illness Bank
Extended Illness Bank (EIB) is authorized for full-time and
part-time employees to be used for personal illness or injury preventing them
from reporting to work. Eligible employees will be credited with the applicable
accrual rate for productive hours and benefit paid hours (regular, straight
time, overtime, doubletime, call back, PTO, sick (EIB), jury duty, limited duty,
bereavement, etc.), but excluding on-call.
Leaves of Absence
Madera Community Hospital offers employees leaves of
absence to attend to personal and family needs. Employees shall provide
appropriate documentation and request approval prior to the leave from their
direct supervisor and/or Human Resources. Leaves are granted in accordance with
Hospital, Federal and State policies and guidelines.
Leaves available include but are not limited to: Family
and Medical, Pregnancy, Military, Workers Compensation, Disability,
Rehabilitation Leave, Jury Duty, Court Attendance, Voting Time, Volunteer
Firefighter, Time to Attend to School Activities, Bereavement, Literacy
Education and Personal leave.
Employees are encouraged to request forms and additional
information from their supervisor and Human Resources regarding opportunities
for leave of absence.
Miscellaneous Benefits
Education Assistance
To ensure that high quality employee performance is
maintained, MCH is dedicated to providing employees with training opportunities
and incentives to enhance knowledge and skills needed to perform efficiently and
effectively. Educational Assistance is available to full-time and/or part-time
hourly employees, with at least one year employment service.
Employee Assistance Program
Employee Assistance Program is offered to all Madera
Community Hospital employees. Confidential counseling and referral services
designed to help employees and their household members resolve personal problems
that may be interfering with work or home life is available.
Employee Health Services
Madera Community Hospital offers the following benefits to
employees at no cost. Pre-employment examinations, preventative immunizations,
on-the-job injury/illness review and follow-up, and health counseling.
Theme Park/Movie Ticket Discounts
All Madera Community Hospital employees can take advantage
of discounts for local movie theatres and local theme parks buy purchasing
tickets from the cashier at a discounted rate. Other discounts for gym
memberships, local discount warehouses as well as for theme parks such as
Universal Studios, Sea World and Magic Mountain are available.
Direct Deposit
Employees may choose to have their paycheck automatically
deposited into their bank account. This service is available with most banks
and credit unions and usually saves the employee on bank fees. Contact your
bank for more information.
Parking
Parking is available to all employees on-site in designated
employee parking areas. A secured parking lot is also available to employees
on-site.
Employee Recognition
Employee of the Month, Employee of the Year, and Employee
Service Awards celebrate hospital employee contributions to the mission of the
hospital. During Hospital Week employees are again recognized and thanked for
their teamwork and efforts.
