COVID-19 Vaccination Interest - This page will be updated and the interest
form activated when we know our next clinic date and time.
If you are interested in and qualify to receive the COVID-19 Vaccine, please
complete this interest form.
Those qualified by Madera County Public Health Officer include:
- All Healthcare workers
- All individuals 65 + in age
- First Responders who are 50+ in age
- All Educational Institution workers
- Ag workers who are 50+ in age.
We are currently appointing for the COVID-19 Vaccination Clinics on Wednesday,
March 3 and Friday, March 5, 2021. We can accommodate nearly 500 appointments
with the first dose of the Moderna vaccine. It is imperative you are available
for the second dose: March 4 and April 2, 2021 - respectfully.
Completion of the interest form does not guarantee a vaccination appointment.
Appointments will be based on first received, first appointed.
We apologize in advance; we do not have the scheduling resources to accommodate
a specific appointment date or time to meet your individual request.
If you have been appointed for one of our COVID-19 community vaccination
clinics, based on sumitting this interest form, you will receive an email
providing your appointment time, the clinic location with map and your
second dose appointment.
Photo identification will be required during on-site check in. Employment
identification may include company issued credentials or pay stub matching
your photo id and birth date. Agriculture labor employers may submit a
request on behalf of their workforce by contacting our Community Relations
Department, WeCare@MaderaHospital.org
If your age or business sector does not qualify based on the most current
guidelines, you will not be contacted and your vaccination request will
not be kept on file. You will need to submit a new interest form when
the tier opens for your age and/or business sector.
